Motor Vehicle Accident Form

If you are involved in a motor vehicle accident where someone is injured, or if there is over $1000 in damage to a vehicle or property, you must submit and accident report to the Registry of Motor Vehicles within five days. Click this link and print out a copy of the form.

Fill out the application and make an appointment with Sgt. Harry Bates. He will go over the application with you.
